RULES AND REGULATIONS OF THE STATE OF TENNESSEE DEPARTMENT OF HEALTH AND TENNESSEE DEPARTMENT OF ENVIRONMENT AND CONSERVATION BUREAU OF MANPOWER AND FACILITIES; DIVISION OF HEALTH CARE FACILITIES CHAPTER 1200-8-1 STANDARDS FOR HOSPITALS 1200-8-1-.07 OPTIONAL HOSPITAL SERVICES (1) Surgical Services. (c) The operating rooms must be supervised by an experienced registered nurse or a doctor of medicine or osteopathy. (d) Licensed practical nurses (LPNs) and surgical technologists (operating room technicians) may serve as "scrub nurses" under the supervision of a registered nurse. (e) Qualified registered nurses may perform circulating duties in the operating room. In accordance with applicable state laws and approved medical staff policies and procedures, LPNs and surgical technologists may assist in circulatory duties under the supervision of a qualified registered nurse who is immediately available to respond to emergencies.

Scope: Tennessee Board of Nursing, Position Statements, 6/87, Reaffirmed 12/01 – Page 9 – First Assistants. The Tennessee Board of Nursing believes the RN surgical first assistant is a category in which the RN functions in a role that is an extension of an M.D. surgeon or dentist and that the RN first assistant is performing a delegated medical function during surgical procedures. To reiterate, we believe the RN first assistant is performing a delegated medical function during a surgical procedure. The Board, in considering the entire perioperative period, takes the position that the pre and post operative instruction of client and family is a nursing function and is addressed in the Nurse Practice Act of the State of Tennessee.
