Congratulations on your decision to start a new chapter. Please use the information on this page, under Supporting Documents, to help you manage the requirements and processes involved with establishing your chapter.

The Chapter Formation Guidelines outline the requirements to form a chapter.You will need at least 15 AORN members (or individuals who qualify to be AORN members) interested in starting a chapter and be willing to submit the following forms available under Supporting Documents.
* Chapter Member Roster
* New Chapter Recognition Form
* Guidelines for Chapter Bylaws
* Direct Deposit Form
* EIN Number (Federal Employer Identification Number)
To help you start getting organized, there are also several supporting documents including
* Chapter Accountability Standards,
* Committee and Officer Job Descriptions,
* Chapter Willingness-to-Serve Form, and
* Document Retention Schedule.
