TICKET TO RIDE
No Admittance to the OR Without This Completed Form
We keep a bright green form by the patient's bedside in pre-op. The form, which we dubbed "Ticket to Ride," is a quick visual verification that the patient is ready for the OR — charting is done, anesthesia has seen the patient and the surgeon has marked the surgical site. The pre-op nurse signs off on the document. There's also a space for our urologists to sign off on ESWL (extracorporeal shock-wave lithotripsy) patients. This serves as a trigger for them to double-check pre-op X-rays.
